Your First Visit

Your first visit in any orthodontic office is a very important one. In our Airdrie office we like to make it as informative and comfortable as possible. At your initial consultation you will be receiving lots of information so it is important that each decision maker be at this appointment.
It has been our experience that receiving the information and asking question first hand is always best. We will book adequate time for you to ask questions and have any concerns you may have addressed.

At your first visit we will be taking a series of digital pictures and any necessary x-rays, then Dr. Liss will complete a comprehensive examination, he will then explain treatment recommendations. Our Treatment Coordinator will review the appointment sequence and investment options with you as well as answer any questions you may have.

Should you decide to go ahead with Dr. Liss’s recommendations we have set aside time in our schedule to start with pretreatment records on that day to save you another trip into the office.

Complimentary follow up consultations can and may be scheduled, to review or answer any additional concerns you may have.

Please help us be prepared for your first appointment by completing these Patient Information, Medical History, and Personal Information Consent forms. Download the forms below (in either Word DOC or Acrobat PDF format) to your computer, print them out, complete the forms, and bring them with you to your first appointment.
